Our initial appointment usually will consist of an examination and review of your treatment options. Please assist us by providing the following information BEFORE your scheduled appointment so we can devote the entire appointment time on you:

-New patient forms filled out online

-Any x-rays taken by a previous dentist, if applicable.

-A list of medications you are presently taking.

-If you have dental insurance, bring the necessary completed forms. This will
save time and allow us to help you process any claims.

IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian.

Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).

X-RAYS

If your previous dentist has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional x-rays are necessary, they will be taken to our office.

WHAT TO EXPECT AT YOUR FIRST VISIT

At this visit, we will take digital photographs of your teeth and smile, along with any necessary x-rays. Dr. Yoshida will take the time to perform a very thorough exam of your mouth and teeth.

Together, you and Dr. Yoshida will discuss your questions and concerns as well as findings from the exam. From the information gathered, we will create a customized treatment plan to achieve your goals.

Our dental team will then discuss fees and financial options with you. And together you will schedule your treatment as planned. It may be convenient for you to have a routine cleaning on the same day. Just ask and we will be happy to schedule these appointments together for your convenience.

By the time you complete your new patient visit, you will already be a part of our family of patients. You will also have an assistant you know by name who will walk you through treatment and be available to answer any questions or concerns.
